Marriage Form A

This is the form to be submitted from your parish to the parish where the betrothal will take place.

Marriage Form B

This is the form to be received from the parish where your betrothel will take place and submitted to your own parish.

Marriage Form C

This is the form to be submitted from your parish to the parish where the marriage ceremony will take place.

Marriage Form D

This is the form to be received from the parish where your marriage will take place and submitted to your own parish.

Prenuptial Questionnaire

This form is to be completed by the groom and the bride and submitted to their respective parish offices at the beginning of the marriage preparation process.

CCD Registration Form

Dear Parents, CCD registration for the 2026–2027 academic year is now open. The registration deadline is July 31.
